Voluntary surrender & hold-harmless — read before signing

By submitting this request I state that I am the registered owner or authorized custodian of the vehicle described above, and I voluntarily surrender it to Recovery Columbus Inc. for return toward the lienholder. I agree to hold Recovery Columbus Inc., its agents, and employees harmless in connection with the pickup, transport, and storage of this vehicle.

I understand that identity is verified in person at the time of the tow, that I (or an authorized adult, 18 or older, with valid photo ID) must be on site with the vehicle, and that the tow ticket is signed at that time.

I understand that if the lienholder does not take possession of the vehicle within 30 days, proper disposal notices and paperwork will be filed to maintain compliance with state and local regulations regarding abandoned vehicles, and that such paperwork and processes may result in charges being placed against the vehicle while it remains in Recovery Columbus Inc.'s custody.
